3rd monitor cannot be duplicated

In CCC I see 3 monitors, but can only hook-up 2 (1+2, 1+3 or 2+3). For instance when I want to duplicate the 3rd, there are only Extend and Replace modes. No Duplicate is shown

sorry, forgot some details.
I bought Radeon HD 7850. All monitors work, but max. just 2. Whichever monitor I want to clone or duplicate as 3rd, does not work in CCC. Have DVI > DVI mon; DVI (I) > VGA and Displayport with active adapter (2nd one) to dvi mon.
I have no clue why I can see 3 monitors, but just 2 I can use at the same time. I play FSX.
Can it be cables, active adapter or software????
