GTX 660 .. Artifacts then Crashes!

Am using EVGA GTX 660 2GB SC the problem is when i play games i get some artifacts breaking the texture of the game and after that it crashes to desktop and then a error come up from the control panel saying "display driver has crashed and recovered" i thought it was the driver problem and then i decided to downgrade to old drivers after doing that same thing happened then i reinstalled my windows after that reinstalled the old nvidia drivers again but still no use .. after that i gave up and decided to look for some answers here .. so guys please help me on this.

My System:
i5 2500k @3.3GHZ
P8H61-MLX
8 GB RAM
CM GX 550 BRONZE EDITION

Temps are normal as before and one more thing when i deoverclock it about 50 mhz everything got fine .. WTF!
 
Assuming the downclocking 50MHz is from its base clock and not your personal overclock, return the card. If it isn't stable at its base clock its a dud.

If you mean you have overclocked it yourself, then going down fixes the crashes, then it just means you've gone too far.

It is a factory overclocked card .. so this means the card is destroyed?
