Solution
List the make/model of the drive. Also check the SMART status of the drive. Sometimes running chkdsk with the /f option can remedy these problems. Also, when is the last time you defragged that drive?

Hitachi HDS721010DLE630
where do i find the smart status? and what is chkdsk with /f?

Here it says it can do 600mb/s? i only get 21mb/s?

alos this keeps poping up, why? :(

chkdsk /f is a command to check your drive for errors and the /f switch tells chkdsk to fix any problems it finds. You should check the Hitachi (now HGST) - http://www.hgst.com/support/downloads/ - site for diagnostic software (Windows Drive Fitness Test) related to your particular drive and this is where you should find SMART status. The popup window you see may be an indication that the drive is about to fail - Windows has detected problems with the drive. You should backup anything of value on that drive as soon as possible.

To run chkdsk, see here. There are instructions for two ways to do this, command line, and GUI. Either will be fine. The procedure is almost identical between windows versions, which you didn't list your OS.

http://support.microsoft.com/kb/315265

Kenrivers listed an app to check the SMART status, "drive fitness test". It definately sounds like your hard drive is beginning to fail. Make backups of your data real soon!

Ive done this twice... but this is what i get:

the chkdsk e: /f and chkdsk e: /r didnt find anything on there as you can see in the screenshot, im a bit lost now :(

please help

From the picture with the command screen, it looks as if chkdsk got hung. It's sitting at 57%. The smart staus shows why it's getting hung. It's stuck trying to read a bad sector and your disk healh is bad. Sometimes a full format of the drive can get rid of bad sectors, but at this point I'd say your drive is starting to fail and should be replaced as it will probably only get worse.

it didnt get hung, i just took picture during process :x

and ive tried to reformat it but it wont let me :(

Were you trying to format your OS drive? If so, you can't do that from within windows. During the OS install you are given the chance to create/delete partitions and also format a drive. Be aware that a full format can take a long time, but it will check for bad sectors during the format and flag them as unusable so that it doesn't try to write data to them.

its an 2nd internal hard drive, how do i re-format that? ive already removed all existing content off it and would like to be able to reuse it as a fast storage device for video recording programs,
thanks

Just so I am clear, you can't format the second hard drive (e:\)? Were you formatting from a command prompt or inside of Windows? Have you tried going into Disk Management, deleting the volume, then creating a new volume on that disk and in the process formatting it?

inside windows, not sure how to delete and create a new volume and format it :p

Click on Start
Type diskmgmt.msc in the search bar
Press Enter
Right click on drive e:\
Click on Delete Volume
After the Volume is deleted
Right click on drive e:\ again
Click on create Simple Volume
You should be prompted to assign a drive letter and format the drive.
