Format the microSD card, and then all files appear again!

I've a microSDHC 4gb class 4 card.
When some files are deleted (it just disappear in windows explorer), if i plug out the card and then plug it in again, that file somehow appeared.
When i try to format the card, windows says that it cannot complete the job. I try with another tools, when the card is unlocked (it locked before format) all files (include files i've deleted) appears and it makes me want to break the card with my hands :fou:  and more, deleted files do not damaged at all, even mp3s can be played smoothly
When i try to partition the card (EASUS partitioner latest free edition) it says there's some error on the card and therefore it cannot be formated.
When i try to check the card for errors, windows says the card is all ok, so do HDTune.
At last i try to do a LLF (it's something mad now), it done and all the files appear again.

What's the problem? I want to format the card. I don't use adapter so there's no read-only lock. I tried with many microsd card readers.

It sounds like The card is no longer able to erase the cells. Burn it/break it/toss it out and go buy a new one.
