USAGE FROM MOST TO LEAST IMPORTANT: Work and gaming. Work wise, I run a multitude of windows and programs. I have 2 screens at my office and I find myself still having to switch and minimize more than I would like. I run spreadsheets, word docs, numerous browser windows, outlook, work programs both java and IE based, and other like items. Gaming wise, I don't know if I truly can see the differences in AA or any of that stuff. With that said, I want smooth gameplay across a resolution of 5760x1200. With my current setup I don't normally kick up any extras but occasionally. I am a late to play gamer. I buy games when they go on deep sales, so I am usually behind, but not by terribly long. I play everything ranging from Fallout(1&2), Kane & Lynch 2, Mass Effect, and BF2. Newer games that interest me are Dishonored, BS:Infinite, and Hawken looks pretty enticing.

ADDITIONAL COMMENTS:
The setup I will be running from my rig will be 3 monitors and 1 TV. The 3 monitors will be in extended desktop while the TV is only needed to duplicate the main display when I decide to play games or movies. The TV will be 3D capable, but I understand that PC 3D games & TV 3D are not on the same field, although I would like to be able to watch movies in 3D.
I am looking at Dell U2412M for the monitors. Newegg has them at 10% off right now. I am open for input on the monitors. My current monitor is an older Samsung Monitor/TV ToC 24" 1920x1200 monitor. I would like to stay with the 16:10 ratio.
Please let me know what items I need to address to not end up throwing money down the drain. My 4890 has served me well, but the breaking point was when I bought L.A. Noire. Booted the game up and got the worst frame rates on anything
I have ever tried to play. At that point I said "oh hell, better do this right".
My wife and I have recently redone our home office and decided to do it proper since we both spend some much time on the computer for work and leisure. That means I can justify "necessary" upgrades!
I am looking for an easy and straightforward solution. I don't expect it to be effortless, but I would like it to be consistent and reliable. Thanks for the info and help in advance.

For 5760x1200 you'll need a monster pc which will cost you a lot of money and forget about you old computer because it's piece of S!#t compared to the one you'll want to build. So what is your budget? (In order to have playable frame rates it must be around $3000-3500!
