PC's USB Ports Broke(?)

I have a Dell Studio XPS 7100 PC. I bought it 2-3 years ago and have made minimal hardware changes to it -installed a graphics card- and everything worked. One day the USB ports and headphone & microphone jacks on the top of the desktop stopped functioning. That was a year ago and I ultimately chose to ignore it after checking device manager and my PC's registry for any issues.

Recently, the ports on the front of the desktop and one of the ports on the back of the desktop ran into a weird problem. Just today, if I connect two devices to the two USB ports in the front, neither of them work. If just one device is connected, it works fine. The same problem occurs with two ports in the back. I checked device manager and it said one USB port had an issue. I uninstalled and re installed it and disabled/enabled it. That lead to temporary fixes but no permanent solution. I'm thinking it is a hardware problem but I'm not sure how to go about fixing this if it is.
