"Com Surrogate has stopped working"

After booting, my PC will give this notification [Com Surrogate has stopped working] intermittently, not every time.
Usually, it does not reappear again after a re-boot.
What is happening and is it anything to be concerned about?

EDIT:
While the notification is showing, all the link icons on the desktop are blank rectangles.
As soon as I click on stop program, they turn to their correct configuration.

I remember having this a few years ago and at that time it had to do with a codec pack I had installed. Have you installed anything like K-Light or such that add codecs to the system?

I am nearing the completion of a new clean install of my OS and apps. This had also been happening intermittently in the old OS install, but had been happening less frequently with time. It surprised me when it began again.

I have not added any codecs yet. While it is hard to ID it to any of the installations, It does seem to have resumed after running all the Windows updates for Win7x64 and Office 2k7.

With that said, I do not use Windows Updates. I do all my updates with WSUS Offline Updates. I can't help but feel that blaming the Win Updates, no matter how they come, is grasping.
