I just built this new system:
http://pcpartpicker.com/user/Locke03/saved/1zyV
and after returning both a bad PSU and GPU I thought I finally had everything working until I went to stress test the GPU. (no overclocking or other kind of altering of hardware settings)

Running MSI Kombustor the temperature quickly spiked to 91c (with the fans sounding like a harrier taking off) and before 5 minutes had passed my display went out and I had to reboot the system. Tried it again and the same thing happened again. It idles at about 38c, and when I put it under load in Kombustor, let them temp rise to about 80c and then stop it, the card quickly drops back to that temperature.

I also brought up The Witcher and ran that for a few minutes with all setting to the max. Frame-rates and everything are good and it didn't crash or go dark, but within about 5 minutes got up to around 80c.

My initial hunch is incorrectly applied thermal paste, but I'm not sure. Any ideas on what the problem could be and how I might be able to fix it, or should I just send it back and wait for a replacement? Or should I just get a refund and go for a different card?

Have you bought any fans for you case? If you have a GPU pushing a bunch of hot air, but no airflow in your case, the hot air is just going to stay in your case..

Try buying a front fan, and bottom fan installed as push(intake). Then add 1 top fan as a pull(exhaust) to your case and start pushing more air through your case. otherwise the hot air just hangs out in your case and heats up all your components and thats not good

I have 2 extra case fans as well, in addition to the fans on the PSU, CPU, and GPU (both of those are working also). All other temperatures inside the case are quite cool. Even the CPU when running Prime95 for nearly an hour never broke 70c.

I read reviews on newegg. A lot of reviews say it cools great, but a lot also say it is awful at cooling.

That card seems to be hit or miss in cooling. If I were you I would probably try for an RMA. You could RMA it for the same card or get a better manufacturer (my 7950 is Sapphire, and it only gets up to 65c while overclocked heavy and mining bitcoins.)

It seems like everything youve done says your system should be running cool. It must just be a defective cooler and they messed something up while putting the thermal paste on or something like that.

I suppose I'll give MSI a call tomorrow (this was the last MSI 7950 at the shop I bought it at) and see if they'll let me re-paste it without voiding the warranty.

Good idea. Just make sure you dont put to much because you dont want that sweating on the components
