Have you tried patching the game? Rockstar has patched this game to 1.0.7, and in their patch notes, it says that the memory management has been improved.

Does this problem still exist when you play on the lowest settings?

Don't feel too bad. Not many people can play this game smoothly without stutters and annoyances. I have a decent laptop made in 09-10, and I can hardly run this game.

 This is probably the most poorly optimized game I have ever played. Supposedly, it is essentially the 360 version on PC; so, it only uses a maximum of 3 cores. And if you have an Intel with HT Technology, the HT cores count towards that 3 core maximum.

I'm no expert, and I am sorry if I offend you, but I just think your system is not strong enough. Even though GTA4 is a few years old, you have to have a good build for it.

I did some research, and people online were saying your GPU is not that great. I would try to buy more RAM. You need at least 4GB of RAM to play GTA4. The processor seems decent for GTA.

no i can run gta 4 with no problem gta lags in car only not on foot its super smooth on foot and todya when i was playing it it was only lagging because of the radio

It lags in car because you are moving through the game world faster, so the game has to pull more data from RAM. GTA IV can easily use 2GB of RAM just on its own. Since you only have 2GB, about 1GB is being used for OS functions, leaving only 1GB at most for GTA IV. At the very least, move up to 4GB of RAM if you want to play this game. I would also recommend a stronger graphics card, but lack of RAM is probably holding you back a bit more at this point.

What resolution is the Window Mode? And what resolution is the Full Screen mode?

Also, I would do what supernova1138 said and get extra RAM. It would also be useful for much more than GTA4.

hmmmm, that's really weird.

I would still suggest getting 4GB of RAM.

And I would say try giving GTA4 a fresh reinstall. And do not use any addons or mods.

I'm from the USA. But I'm full-bloodied Albanian.

You should be able to run Battlefield 3 on the lowest settings. You might be able to run it higher.

Here are the games requirements:

From what I heard, Battlefield 3 is better optimized than GTA4.
