Can someone please check to see if there is something wrong here

I know this is probably not enough information but I have been struggling with BSOD (BLACK screen of death with reboot) 20 minutes into playing games for a while now.

Did a fresh install of windows the other day and got to play a game for 40-50 minutes before i got something that looked like a green screen with vertical lines and every fan went nuts!

So im thinking this is either my graphics Card or my PSU. But I might have some wrong settings so I am reaching out to you guys to see if you can help me out.

Hey, yes this was idle and the 4600 is the Liquid Cooling

"12.67-12.69 on your 12v is outside of ATX spec which would make me wonder what the regulation would be when its being pushed by your CPU & GPU" see this is where i think the problem rests. because from what i can gather this is where the problem starts, once everything really starts booting up. Is there someway to check this? Should i try bios and manually set somethings?

Any suggestions are accepted with gratitude!

My PSU is the SilverPower SP-S750M 750W

I have been monitoring the heat ever since this problem started (couple of months back) and the CPU is cool and calm around 35-40 with heavy load, and the GPU never goes over 60 under heavy load it usually stays around 50-55 while gaming and this is why i also suspect my PSU is the problem somehow.

I had a 450W PSU that i exchanged for this PSU and I am in the process of finding out if it is something wrong with the setup or if it should be RMA

Well the other PSU i exchanged after i bought a new GPU in so that I would have enough W to not deal with problems like this.

it is weird yeah. I am trying to see first if there is something i can try before I RMA it, like fiddling with the settings on voltage etc. that might be causing this.

That's true, I'll leave this here for a few days just in case someone stumbles upon something amazing :D and I'll update again if I need to RMA it. Thanks man!
