Random black screen crashing

I've been having random black screen crashes in Windows for the last month, often when the system is not under heavy load and the temperatures are low across the board. The computer will just lock up with the screen turning black. Restart is required. If it was playing music when it happened, the music will still and repeat a buzzing/humming sound. Sometimes the GPU fan will kick to 100% when this happens.

Lately, the issue has become reproducible if I invoke the graphics card under certain circumstances. For example, starting EVGA OC Scanner and clicking the benchmark button will cause the system to immediately crash. Other times, though, I can play video games for hours without issue.

I've already replaced the graphics card with no change, as well as cleaned the drivers from the system multiple times. I ran memtest86+ for hours without any errors. Also run Prime95+ with the temps going into 70C+ with no crashing.

My first thought was the PSU but it was replaced not that long ago and 850W should be more than enough for this system.

Any ideas? I'm stumped on this one.
