Crash, then disk read error

I get random crashes from my 250gb SSD, followed by the "disk read error". I have to manually turn off the computer using the switch. I have a 500 gb HDD installed on the mobo as well. Should I just image the SSD (windows 7) using Acronis to the HDD, and reinstall Windows, or should I image to an external drive? Can I just change the boot order in the Bios and run the backup drive? If I use Windows backup is that the same as Acronis? Do you think my SSD is dead? I ran a drive health program and it seemed fine. I ran chkdsk, replaced the mobo battery. This game I play, (Dark Souls) is a console port and really buggy, which I think is the root cause of my problems, it crashes constantly.
 
When you say "crashes from your SSD", do you mean just that your computer crashes when playing that game, or did you test things and figure out it was our hard drive

If it's the game that crashes, installing Windows on a different hard drive won't change anything. Re-installing Windows won't hurt things though aside from having to re-install the applications and migrate settings over, so may as well try that. That way you can also work on the system for a while and see if it's stable outside of that game crashing.
