Screen blacks out in the middle of my game

Since a while ago I started having problems when playing League of Legends.
I don't have ANY lag at all during the game, I'm on a steady 70 fps on the highest quality settings but I often get a blackscreen in the middle of my game.

It goes like this:
Screen goes black
Few seconds later I get the message saying theres no video input from my monitor
The message goes away and its just black again
And another few seconds later I see everything again and everything is back to normal.

Sometimes it happens 3 times in a game, sometimes it never happens.

When I get the black screen my youtube video stops aswell (no sound) but my skype call still works.

This is why I'm pretty sure it's an issue with my GPU..

My CPU used to be overclocked to 3,4 Ghz but my previous hard drive crashed about 4 months ago and somehow it got reset I believe, might go OC it again soon.

Also sometimes my hard drive makes a very loud sound when I for example go to my program files.

OS: Microsoft Windows 7 Ultimate 64-bit

MB: Gigabyte Technology Co., Ltd. EP35-DS3 (Socket 775)
CPU: Intel Core 2 Quad Q8400 @ 2.66GHz
CPU cooler: Cooler Master Hyper 212 Evo
GPU: Gigabyte GV-R777OC-1GD (rev. 2.0)
HD: 932GB Western Digital WDC WD10EZEX-00RKKA0 ATA Device (SATA)
Screen: Philips 227EQPH (1920x1080@60Hz)
PSU: Corsair CX430

If anybody knows what might cause this please give me a reply :)

Update:

I thought the new driver fixed it but the problem started again yesterday somehow.. the temperature isn't the problem so I really have no idea anymore..

yep its definitely gpu related, try diffrent drivers ( get the newest ones, if you already have the newest ones, go back on driver).

also, if your pc is old, your gpu might be overheating which casues the driver to crash, so download a program called HWmontior to montior your temps, and check on google the max allowed temps for ur gpu.
and hard drive making loud noises is fine, hard drives failing is very rare.

My PSU is Corsair CX430
I have editted it in the main post.



And I'll go check if the if it overheats but it's not really an old GPU at all and the cooler on it seems pretty good so I doubt that is the problem. Also when I check the temp of my GPU its never really high but I haven't checked when I'm for example in game for a long while so I'll check that and see if theres any updates for my GPU.
