best low profile fanless GPU needed

I'll have a Dell 9020 SFF delivered soon and need to get a GPU for it to play games.

I need a GPU that will be low profile and another restriction is the PCI slot leaves no room for a large heat sink or fan on the side you'd usually have it.

Whats the best thing I can do GPU wise in this situation?
Can you get powerful cards with the heat sink fan on the other side?
Can I convert a card somehow?

ok, so I've put in my old Radeon HD 5570 low profile card from my old pc and space is very tight.
So tight that the small fan on it blows against the side of the PSU (about 5mm away) which speeds up a lot to move the air making it very noisy. This isn't going to be good for removing the heat either i'm sure.

I'd like a low profile, single width GPU with passive cooling as powerful as available if you have any suggestions. Its a big ask I know.
I have seen some after market heatsinks that bend around both sides of the card so would look at that option to but don't know much about them.

If you come across one of those passive heat sync cards that extends around the back of the card, please post a link. I'm curious as to what it looks like.
Does your Radeon card take up more than 1 bay width? I can't visualize how it could be that close to the PSU. (Btw, the cards fan is an intake, not exhaust. Not that that matters... )

If possible, please post a pic or two of the situation.
