EVGA GTX 670 FTW edition was working now not!

Hi all
before i begin i would like to point out i did ask a question about 2 weeks ago and everyone said that my gear can runthe card fine.

and so yesterday finally got my new graphics card (brand new unopened)
installed properly with update to nvidia driver rebooted installed evga precision x, and rebooted and the when I was certain it was installed correctly, ( watched you tube videos to be safe, i have installed g cards before just wanted to be safe.) i turned on FRAPS to see if i was getting any bottleneck and run Crysis 2, put it on 1920 x 1080 on ultra with testalation pack ect and of course was running like a dream i was pretty happy :) .

i then ran Chivalry: Medieval Warfare on ultra highest settings on everything again with fraps and had the same fun, lots of pretty light shafts and full detail ect everythng was running at avg: 60fps
max :70 min;36 pretty average high detail fps i would think. but heres where the trouble stats. I alt + f4 out of chivalry and then went to tomb raider, it started to load and then my monitor came up with its osd saying no signal detected and went into sleep mode. i just thought "oh, just a crash ok". i hit the restart button and it booted fine until it displayed and i noticed the screen size was only 800 x 640, and once the windows loading had come up many white horizontal line have come up all over my screen. i went to nvidia control panel and it said that no monitor is conected to nvidia GPU.

so i went to device manager and it tells me that it has error code 43

i have tried

1: re installing drivers, 7 - 10 times i've lost count.
2:updating BIOS
3: completely removing any trace of nvidia drivers and yes including registry.

I put the old gcard in and the screen is fine it runs windows @ 1080p like normal.

so i'm asking.
1: Is this a problem i can fix?
2: Should i just return and get a new one?
3: does the fact that it was working and now doesn't mean its just a faulty card and i'm just the unlucky one?

i'll also add the old gcard DOES NOT display the whit lines.

the white line are randomly gaped and run in columns that are off set they are not parallel kind of like this: and it runs through text so i sometimes have trouble reading

It likely is. If you feel comfortable sending it back, then go for it. I only say to check things like the PSU because of the off chance that you get a replacement card just to find out that it does the same thing.

Finally, I checked in with an old driver version 258.96 (with old installation wizard) and does not display CODE 43 in Device Manager with low-res desktop. But now its does artifacting on desktop! I try with the thermal compound (the recommendation to take the heatsink of the card, remove any remains of old thermal paste and clean the GPU and heatsink contacts properly and apply a fresh layer of thermal paste). I conclude from this that the new versions of drivers are with the built-in diagnostics for health of card and any type of error happens it reports CODE 43 to stop the operation of the device.
