Gigabyte Radeon HD 7950 Crashing During Simple Tasks

First off, my setup:

Intel i5 3570K
8GB Ram
Gigabyte Radeon HD 7950
ASRock Z77 Extreme4

Previously I was running my desktop on my HDTV, I had some "driver stopped responding and recovered" errors, but once I fixed the scaling in CCC it worked fine without any more crashes.

Now I have switched to a computer monitor (non HD) running at 1028x1024 resolution and I get constant crashes. It crashes literally every time I click play on a song on SoundCloud.

No overheating, I've tried uninstalling and re-installing drivers, switching GPU slots on the motherboard, neither worked.

Any other ideas? My final idea is to reformat but I really want to save that as the last decision. This card has given me so many problems it's unreal.

Have you checked if it is overclocked, reset it if so. If it is running at stock, I would RMA the 7950. A format is always worth a try should that fail.
