Should I let this HDD Die now?

Its giving me random times of slowness and just weird running issues in general. Nothing I can pinpoint other than the slowness especially during file transfers.

Here is HDTune. I mean it has almost 33k hours of use which is about 3.75yrs 24/7 usage...Is it time to send it to the grave?

Solution
Check error with hdd tune to determine how much bad sector on your hdd..
bad sector or delay sector on your harddisk can cause very slow and unstable transfer rate..

if you found any, then you may try "HDD Regenerator"(http://www.dposoft.net/hdd.html) to try fixing any bad sector and delay sector..
Well, it's works almost like magic..clean my "almost damage" harddisk with 30% badsector completely.. you may try your luck too..:)

And don't forget to update your chipset driver to latest version..

The HDD is still OK, but like you said it's got performance issues. That said, I wouldn't throw it away. What I do with my old HDD when I retire it is put it in an external enclosure and use it for a backup drive (replacing the drive that's in the enclosure already), while only turning it on when needed.

This allows you to still use the drive, but in a way where performance is not as big an issue.
