Graphics Card Not Working properly

This one is confusing...at least for me it is. The problem im having is that when i play games it lags a lot. Games that ive previously played without any fps issues. Whenever I click the nvidia settings it says I dont have a graphics card, yet my monitor is hooked up to my card and the image is being displayed fine. When i start the computer, in the bios it recognizes the card but windows doesn't. I think the problem might be windows but i wouldnt know how to go about fixing that.

Ive tried system restore.
Ive tried updating my video drivers twice
I tried taking the card out and putting it back in, but nothing fixes the lag.

After doing more research within windows it appears i have a code 43 on my video card.

Run DXDIAG and post screen shots.
Check for [!]'s in Device Manager and post screen shots.
