Multiple crashing issues

Alright, I have a multitude of problems/crashes that seemed to be linked to my video card. I've been trying to play Skyrim for a few days now and I keep having these problems: When I get into the game everything is fine as long as I'm indoors. I can do all the normal stuff in the game. Seconds after stepping outside I see these tiny little glitches that sort of pop up for a split second in different parts of the screen. They're mostly grey triangular type shapes but occasionally they are a different color. After that happens, one of 3 things happens. A. My computer freezes for a second and skyrim closes. B. My computer black screens for a few seconds, skyrim closes, and I get the error "Nvidia Kernel Mode display drivers have crashed and recovered" or C. My computer screen goes black and I can't do anything until I reboot.

P.S. Yes I have tried updating my drivers, doing a driver sweep and updating, etc. It's not a virus either. I've run a multitude of tests for certain viruses and I'm clean.

P.P.S. I also have another odd occurrence. Sometimes when I launch my skyrim none of the above problems occur. Instead, the game is terribly laggy as if somehow my card was underclocked without me doing anything.

Honestly based off what you have told me I would think so. The best thing to do, would be to download a program like MSI Kombust and let it sit and run for a little while on full screen mode. If it doesn't crash, then the card is fine and its another issue, but if it does then I would take it as a confirmation that your card has gone bad.
Are you overclocking? It sounds like you are pushing your card too hard and that it cannot keep up. Those are the same kind of things you see when you overclock a graphics card too far.

If not, then I would try cleaning out your PC really well to make sure there isn't a dust build up, or try adding a case fan. If you are not overclocking, the only other possible thing I could think of that would cause issues like this are overheating and card failure. Unless you sill have warranty you cannot get past the card failure.

I am not overclocking. I cleaned out my PC with an air compressor and it still hasn't helped. I was just playing Skyrim again and I listened to how my computer sounded while it was playing. There was a high pitched sound that only stopped when i went into the pause menu and when the display drivers crashed and the screen went black the fan started going much louder while the crash was happening. Does this mean my card is screwed up and I need a new one?
