Machine freezes with PCI-E video card installed

OK, so here's the story...

HTPC build with the specs listed below suddenly will not run any OS for more than 30 seconds with a video card installed in the PCI-E 2.0 x16 slot...

MB: Gigabyte GA-B75M-D3V
CPU: Intel Core i3-3220 Ivy Bridge 3.3GHz LGA 1155 55W
RAM: G.SKILL Ripjaws Series 8GB (2 x 4GB) 240-Pin DDR3 SDRAM DDR3
HDD: Seagate Barracuda STBD2000101 120GB SSD
Video: eVGA Nvidia GeForce GT620 low profile
Audio: HT Omega Striker 7.1

It boots fine in Safe Mode on Win7 32-bit, but will not boot Win7 or Win8 normally... I've eliminated the possibility of BIOS, HDD and video card issues... I'm at wit's end on this thing. The MB is past free replacement, so I'm hoping for some magical advice before having to replace it... any ideas?

whats the psu and is it old? does it just crash completely after post?

also try another psu if you have one.

The PSU is a Thermaltake TR-450P 450W ATX 12V V2.3 & EPS 12V 2.91 80 PLUS BRONZE Certified Active PFC Power Supply... it's only been in operation for about 8 months and it doesn't crash directly after post... I'm able to launch BIOS and Windows Safe Mode without any problems.

grab hwmonitor and check the temps on that 620 it could be overheating.

I just installed the GT620 today thinking the GT430 was fried... still no luck... Any ideas on how to test the function of the PCI-E x16 expansion slot?

if you have another psu at all i would try it with the pc and the card in the machine.
