Ghosting and horizontal lines

My monitor has a lot of ghosting and grey horizontal lines across it. What's causing this? Monitor about 4 years old - used for work - running Windows 7 OS.
How can I put it right?

-Do you use some sort of adapter such as VGA to DVI or DVI to HDMI, etc.?
-Did this just start happening suddenly?
-Did you recently install any new hardware on the PC or near the monitor and PC? Sometimes interference with another device can cause this. Check for any power or USB wires near the monitor's wiring. Do you bundle your cables together with twist ties or zip ties?
-Check for any loose cables especially the DVI, VGA, DP or HDMI. If it is DVI or VGA make sure no pins are bent or missing and that it is secured by tightening the thumb screws.
-Does it go away if you switch to a different PC (if you have access to one, try it)?
-Did you recently change any settings on the monitor itself or on the PC such as resolution, refresh rate or graphics drivers?

Lots to answer here:
1. HP 2009v Series Wide LCD Monitor
2. 1600 x 900
3. don't think I use an adaptor - how would I know?
4. It has been doing this for some time but I just found this website
5. I have had the computer tower and monitor with peripherals for over 4 years - I don't do gaming.
6. I have a Canon MG8250 printer about six inches away from the tower and about 18 inches away from the monitor which is about six months old - I had the ghosting and horizontal lines long before I changed printers
7. I have very limited space on my desk - there is a multi-socket just behind my monitor with 5 sockets in use
8. I haven't moved either the Tower or Monitor since it was purchased in 2009 so I can't see how any cables could come loose.
9. I don't have another PC
10. I regularly check the resolution - just in case this will sort the problem.

11. I don't know whether its DVI or VGA - probably the latter - how do I find this out?
