Is my gpu done for? (Driver Crashing)

About a week ago my computer started to crash. My screen freezes and then looks something like this http://goo.gl/kXyyu0 Most of the time about 5 seconds later the screen turns black and I have to hard shutdown my computer. Sometimes it goes back to normal and just says my driver has crashed.

I upgraded to the latest driver and that didn't work. I tried the beta driver and still nothing. I then tried Driver Sweeper to get a clean install and that didn't work either. I checked my gpu heat and it was around 35-40 when I crashed. I cleaned out my case of all the dust.

I can browse the internet and watch videos all day and nothing happens. It only crashes when I play games. Any idea what it is, I'm really hoping I don't need to buy another gpu. :( I should probably mention that i've had this PC for about a year now and this is the first major problem i've had.

gpu: Nvidia GTX 660
cpu: Intel i5 3570k not overclocked
MoBo: MSI z77a-g41

Just an update, I updated Bios which didn't help any. Also, I ran FurMark to test my gpu and I crashed within 5 seconds. I also ran Prime 95 to test my cpu and everything went fine with no crash.

I bought it from Newegg and the rma from them is over. I'm not sure if EVGA's rma is valid if I bought it from Newegg.
