Gtx 460 sudden artefacts, black screens, fan dies - stress tests: OK. Anyone??

So I tried several games with same results, I get some "tears" appearing on screen, black lines, flashing triangular stuff and then sometimes the gpu fan dies, screen goes black and a second later the fan and screen starts working again but I still have artefacts.

I can't really reproduce the issue. Sometimes everything works fine.

I did MSI kombustor burn-in test for 15 minutes with no problem. 77 degrees C max gpu temp. I also did (I think) all of the other tests in that program.

I did 3Dmark's free tests (thrice) which checked out fine aswell, no issues.

But then I run Skyrim, Dota 2 or path of exile and the issues start appearing. But only sometimes.

I should add that at first I used to get "driver has recovered " windows error when the screen went black but I don't anymore.

I've run the same setup for 1,5 years with no issues.

I've done extensive malware and virus removal work, following malwarebytes guides. Didn't find anything.

GPU temps are typically 40-45 degrees C idle, 65-78(max) load.

computer specs:
i5 2500K cpu oc 4,5 ghz (this has been stable for 1,5 years)
4gb ram
500w psu
gainward geforce gtx 460 1gb physx cuda
Windows 7

Ok so it's been a few days and I used driver sweeper and reinstalled the drivers from nvidia's home page.

I had no problems whatsoever for 10+ hours, then I had a very minor graphic glitch and short lock-up of the computer so I ran MSI kombustor again which resulted in my monitor loosing signal and I had to restart the computer.

After that I had 10+ hours of up time with no issues until I had the old artefacts come back for about 2 minutes and then disappear again. Now 3 hours later they haven't come back.

What gives?

I really tried searching for info on this and I guess it's probable that my graphic card is dying but I haven't found anyone who has such intermittent and varied problems and I don't want to get a new card if it's some other component that's broken.

Anyone?

Fan is fine! I've cleaned it and there's no dust or anything on there!
