I have a radeon 4800 and on some hidden object games (casual games) the fan it at a very high speed. I wonder if there is anything I can do about it.

Clean the fan and check the airflow in your case. Maybe cables are blocking the airflow.

If you don't already, clean out the dust inside of the computer. Using a can of compressed air or an air compressor and using short bursts, take the computer outside, open it and blow it out. NEVER use a vacuum! Dust acts like an insulator and needs to be cleaned out about once every other month on most machines, if in a area that collects dust quickly, then more frequently would be necessary.

I have clean it out and there isn't any cables blocking the airflow since the fan is facing downwards. Most times the fan runs quietly but not with some of the games.

Sound like simple heavy load on graphics intense games.

Try watching the GPU temps and usage while playing those games, the fan automatically speeds up to keep the GPU at certain temps, if a game is taxing it more, it will need more cooling.

Also, make sure you are using the latest driver from AMD.

I checked and yes I have the latest driver for the card. I wasn't sure if other people have the same issue or not. I wonder if it was a new virus. Is there any way I can tell if I should be listening to hear if my video card fan is going to break down? ((I might have to post that question in a new thread.))

Most of the time, when a fan is breaking down it gets noisy, even when not under load. Otherwise, keep an eye on the RPM's and see if the fan is getting slower at idle (over time). Also, your temps will start going up and the fan will try to spin faster more often.
