Computer Hangs/Does Strange Things

I've been having some issues with my rig and I'm hoping I can get some help here.

It started with my system running really slow and hanging frequently. So to troubleshoot I've run Memtest for hours with no errors, checked for corrupt system files, I've run Hitatchi disk check with no problems, I've used 3dmark11 with my video card never having an artifact, I've run prime 95 for hours without issue.

So I figured there was something wrong with windows -- I re-installed it. After re-installing/formatting my hard drive, it seemed to run quite peppy for a bit, but now when I launch programs it will sort of hang, where I can move the mouse on the screen, but nothing else works unless I leave it for a few minutes, then it runs fine again.

One day it didn't wake up from sleeping, which was a problem I had before that I remedied by putting a new PSU in, so I did that. Hardware monitor also showed my 5v rail was low (4.8v) after the new PSU it's at 5v.

Then today I went to rename a folder on the desktop and Windows renamed like 5 other shortcuts the same name eg: Folder (1), Folder (2), Folder (3), etc.

I'm sure something isn't right.
