I don't know how to describe these. I tried to search and get to know this problem but all I could find is about screen flickering. From my visual understanding (I'm a noob), I can't consider this as flickering so I posted here the screenshots and hopefully I will be enlightened what the problem is/are, what to do, and what I shouldn't do.

This problem only occurs after an error (atikmdag). I have searched about it and followed the steps (install fresh OS, drivers, use driver sweeper (safe mode), cc cleaner, reseat the gpu, rams, cpu, check temps) to remedy it but to no avail.

CPU specs: (all default, I have never OC'd nor UC'd)
i5 3570
Asus Matrix 7970 Plat - 2 months old
Asus M4extreme - 1 year old
Gskill 2x8gb f3-12800cl9q-16gbzl (only installed 2pcs on the recommended slots) - 1 year old
1tb black caviar - 6 months old
1tb blue caviar - 3 months old
x750 seasonic - 2 months old
swiftech 220 - 2 months old
haf xb - 3 months old
samsung ta350 - 1 year old

Other steps taken:
replaced the hdmi cord
tried all the sockets in the gpu
memtest86 (all 4 have 0 errors)
furmark (max temp is 80c, no visible screen errors unless atikmdag pops up)
used only 1 ram (atikmdag still happens)
edit the TDR value

PS:
My Nvidia 570 gave up on me 4 months ago. Before it did, it was already popping up the same error as atikmdag - graphics stopped responding and recovered.
I live in south east asia, I hope that clarifies why it runs 80c at full load in Furmark.
Its idle is 30c-35c whilst cpu is 25c-33c at idle and not more than 55c on my personal full usage.

Make sure you have the latest graphics drivers.

Also, does this happen in other games?

@Deemo. Yes, sir. I have 3.8beta for my gpu and the I installed the latest drivers for my mobo's chipset etc. It happens in other games as well even when I posted this thread hours ago via google chrome. But these kinds of visual problem only happens after atikmdag occurs. Sigh. I'm about to give up on PC. I should have bought laptop instead.

13.8beta*

I'm guessing you've reinstalled drivers already.

Is the card within the return date? Also, did it come factory overclocked?

Yes I have reinstalled the drivers that came with this. I also downloaded the latest driver I found in Asus support as wel las in Amd support. Yes it is factory overclocked to 1100. Not so familiar with it since I don't usually tweak.

You might want to try and find what the base clock is, and clock down to that.

Do I have to lower the voltage too?

Try to search the web for the stock clocks for the reference (base model) card.
