Monitor with tons of different color lines? Please Help! (Video Included)

Hi, i have had my pc for almost a year and in the last few months i have been experiencing a problem, my screen will have all these lines on it and i cant do anything on that screen. The computer is still responsive and i can use my other monitor but if i unplug the monitor thus making my second monitor the default or first one it also happens on that screen, so it is not my monitor. I have re-installed drivers and it seems to help for a few weeks but it still is a problem, just happened a few minutes ago, does anyone else know how to fix this annoying problem?

I forgot to add that it happens when opening a video 75 percent of the time, happens a few hours after use.

what is the temp of the video card, and it would help to know what equipment you have, including how airflow flows through the case - i'm guessing it might be a heat issue, if it's not a cable

you already diagnosed that port 1 on the gpu (or port 2-not clear) is going bad by swapping cables to the other monitor and it still not working

I am running a Phenom x4 965 BE and a HD 7770, the motherboard is a ASUS M5A78L-M LX PLUS. Tempatures have always been below 85 while at heavy load on the GPU and never go over 70 with CPU. I use a DVI-D and a DVI-I port, whichever one is listed as monitor 1 has the problem.

ok, if you have the latest video driver, i would RMA your gpu because it seems somethings messed up with it

Still happening, if I Alt+F4 out of the video it is fine agsin, very strange. I also re-installed windows and that did not fix it. :s
