Facing random Boot issues after installing Graphics Card on ASRock Z77 MB

Hi Guys,

This is my first thread on toms hardware, so if I miss out anything that is required, please let me know. My PC specs are as follows:

Processor: Intel i5-3570k
MB: ASRock Z77 Extreme4
RAM: 2 x 4GB Corsair Vengeance 1600 Mhz
PSU: Cooler Master 350 Plus
GPU: ZOTAC GTX 650 AMP! (non-ti) 2GB (Installed about a month ago)

I assembled this computer almost a year ago (except the gpu) and it was running smooth without any issues. Its only after I installed this graphics card that I have started to face this weird and frustrating problem.

When I first installed the graphics card, my PC wouldnt boot and threw a dr debug error code d4 ("No Console Output Devices are Found"). At first I thought I need to switch to my discrete graphics card in BIOS but then that was not possible as the PC was not able to even POST. So I just removed the graphics card and plugged the DVI cable onto my onboard DVI port. Horror of horrors, the PC would still not boot (same error code d4!). I was petrified thinking that inserting the graphics card may have short circuited my MB due to low watt power supply. So I reinserted my RAM but still no luck. As my last resort, I thought of reinserting my proc. And would you believe? It booted! I quickly entered the BIOS and viewed the system browser. And there it showed my pci-e x16 slot occupied (wtf!). I again installed my gpu and it booted without any issues. Without further ado, I installed all the required drivers and the system has been running smoothly since, until recently.

At times, when i start my PC, it will throw a dr debug error code d4. After a reset, the error codes occur in the following sequence d4 > d6 > d7 (always gets stuck at d7 - "No Console Input Devices are Found" - even though i never removed my keyboard or mouse). The most baffling thing about this issue is that no matter how many times I reset, clear the cmos or reinsert the RAM sticks (even tried with just 1 stick), it only boots up after I reinsert the processor. Doing that everytime is a pain in the neck and not practical. This is the most craziest issue I have ever come across in my life making it next to impossible for me to solve. Power cycling the pc also does not help. Is this a known issue?

I have spent a lot of time searching for a solution online but unable to find one. Please help me resolve this issue. If any doubt please ask. Thanks in advance.
