Problem which i believe is a bad graphics card

Okay, so i'm having a problem figuring out what is causing all of the following:

1. Blue Screen 0x116 atimpag.sys (VIDEO_TDR_ERROR) (Tried reinstalling many different drivers for like 40 times, every time following a guide to remove every last folder and registry key there is from amd)

2. Sometimes (Randomly, not everytime but like 1 of 3 or 4 boots) the display is flickering and has few vertical lines which makes everything unreadable.

3. Sometimes when booting or waking up from sleep, the monitor doesn't wake up while computer does and i have to make a cold reboot (sometimes once sometimes 3 times) for it to go on again.
I doubt it's cable problems as it only occurs in these cases.

I've ran memtest 86+ for 2 times through without errors and windows memory diagnostic tool once without errors, so i eliminated RAM being at fault.

I've run some HDD check programs and they say that it's good and fine.

It is not an overheating issue as i've tried monitoring that too, and the temps dont go high as i spent some nice money on my cooling.

I removed my GPU Today and have been playing around with my 3570k's Intel HD 4000 iGPU for hours now without any problems at all, this and the problems 2. and 3. is leading me to believe that my GPU is a failure and i should RMA it. But i want to ask opinions before sending my GPU back. It wouldn't be cool if i sent it back and it appeared to be Mobo or PSU issue.

Could something else still be causing all of that?

ASRock Z77 Extreme4
Gigabyte Radeon 7950
8Gb Corsair vengeance memory (though playing around with 4gb now, since i borrowed another stick to my friend. but the problems occured even before i borrowed him the stick)
XFX 550w Core Edition
Samsung 1tb 7200Rpm HDD
 
You have done just about everything that you can to determine the problem and the results do point to the video card. If you have a way o test the card in another computer or bring it to a nearby computer shop and have them test it before sending it back for a RMA.
If not the RMA process usually requires that you get a ticket number from their tech support so you will be able to have them run through things to try also.
