Basically my computer that is just shy of one year old was working marvelously, and then I packed it back up into it's box it was shipped to me in, and moved across the country, and now it's all sorts of screwed up.

Initially, my computer was getting pixel lines, and the screen image was all broken up, nearly immediately upon booting. Computer booted, I tried to open a folder, and the graphics fell apart, screen would freeze, and monitor would go into standby and not recover. I tried two different monitors, two different cords and it happened with both.

Simultaneously, my HD started making clicking noises right before the screen would fall into pixel mayhem.

I have updated my drivers, reseated my RAM and video card, updated windows, and cleaned up my registry files, I had a few small registry files that were broken. Defragged, cleaned out old files, and now I can use windows. I can post on the web, watch videos on youtube, and open folders without it crashing, it does however, hang and 'stop responding' for a bit, which it never did previously.

I cannot however, load any games, it will start to load, and then it times out, it may recover one time, but not a second time, then the monitor goes into standby mode again. If I put the game into the background and come back to windows, then the game will hang and not load, previously it would load in 5 minutes, but never loads now.

And on top of it all, I just got an error that my HD has problems, and I need to replace it. It has reallocated one time, I have 5 partitions total.

All around, it's a giant mess. I don't know if I need to replace my graphic card and my HD, or just my HD. I don't know if there something else wrong that is breaking everything else in my computer. The HD issue is brand new, the monitor issues started several days beforehand, did that cause the problem? If I fix my HD will my video card work fine again? I would also take suggestions on what parts to buy that will work with my other components as well.
