Graphics card broken or something else?

Hi.

My graphics are failing pretty severely under stress, which you can see in this video http://youtu.be/i0dnUZTcKz4.
Now it's obvious that something is broken and needs to be replaced, but I'm just looking for a little certainty on what actually is broken.
From this video, would you say that it's the graphics card that's broken or maybe something on the motherboard or the PSU?

Specs
i5 2500
XFX HD6870 Dual Fan
Asrock H61M/U3S3
XFX Pro 450W
No OC's anywhere

I've already updated every possible driver and flashed the latest BIOS.
Temperatures are, and always have been, fine. CPU and GPU don't exceed 75 degrees under load.

Hi, have you tried doing a clean driver reinstall? What graphics card is it? Have you got any overclocking on any of the components?

Also, can you post all specs of your computer?

Haven't done a completely clean driver install yet, will do that when I get home, but I have tried various driver versions including 13.10 beta. All have the same issue.

I posted this in a rush because I had to leave so I forgot a lot of information.
I'll add more info to the question now.
