Okay, I've checked the forum and this one is not out there. I have had a Maximus Formula V for about a year and it has been acting a bit flakey at times. I've noticed that this occurs when I have my Quadro 6000 card installed. Running two GTX 480s is not an issue, just when I have a GTX and Quadro card installed. At first post I will have no video output. If I reboot I will get no OS error. I'll have to go into the BIOS, hit the SATA setup screen, hot swap the OS drive, then save and reboot. Pretty damn annoying. Oh and my first Quadro burned out and had to be replaced. Any ideas? Here is my setup:
Thermaltake Lvl 10 Snow Ed, ASUS Formula V,Quadro 6000, Intel i7 3770k OC
4.6GHz,Avexir Core Series 16 GB 2666MHz, Corsair H80i CPU Cooler, Corsair
HX1050 Pwr Supply

Do you have the latest BIOS and mobo drivers, my first thought is it may conflicting drivers, second thought is the PSU, think the 6000 can draw up to 230 watts by itself, the 480s in SLi up to 625 or more (depending on where you read), might want to just try running the Quadro on it's own PSU and see if problems persist if the other suggestions don't pan out
