Strange pixel artifacts

I'm having a strange issue with a pattern appearing in games and websites. It's difficult to describe. It's a pattern of about 8 dots clumped together in two rows. These dot clumps are then kind of checkboarded. Here's some pics:

It's actually only happens in a few places. I've seen this on the Gamespot forums, on the Netflix video player UI (the place where you click play and it shows the title of what you are watching), and in Borderlands 2. Specifically in Borderlands 2 only on the character model, both on the menu screen and the pause menu like above. I don't think I've seen it anywhere else in the game. The pattern flickers so it can be hard to tell. Outside of BL2 it's static. Lastly I saw this pattern on the Firefox tool bars when browsing themes. Once I chose a theme and installed it, the pattern on Firefox's UI went away. These are the ONLY places I'm seeing this happen.

This is a new build, about 2 months old. The GPU is EVGA 650 ti Boost, their overclocked edition. I am using Nvidia Experience with the most recent driver. This actually updated last week I believe. Prior to BL2 I was playing Crysis 2 and Dead Space. Neither games had an issue. Is this a graphics card issue?

Solution
go on the net and look for klite codec pack. install that. for the game......... could be something as simple as a setting in the games control panel............ or the card is starting to fail. thought the under clock might help.......... especially lowering the ram speed. is there adequate ventilation in the case?

update adobe flash player.

Underclocking makes no difference for the gamespot forums and Netflix. Borderlands 2 isn't showing the pixels, but it wasn't showing them today prior to underclocking either.

The card runs 60 degrees C. I'd say yes there is adequate ventilation. There's 3 case fans, one intake on the front and an exhaust on the back and top. Temps are good so shouldn't be an issue there.

I updated flash player and installed the most basic of the klite codec pack. That has fixed the weird pixels on the gamespot forums and the Netflix UI. Thanks!

The problem in Borderlands 2 hasn't been present today for some reason though. As of this moment, I'm no longer having issues.

Alright so I shut down my PC and this morning I boot up Civ V and I'm having the same problem in that game. So I close out of that and check gamespot and Netflix and those sites have the pixels again too. I tried to repeat steps from yesterday but that didn't work. I didn't redownload klite though as it's still on my PC. I did try to update flash again and I underclocked the GPU again.

Yeah unfortunately I have no way to test the card or a different card to test in the MB. I suppose it could even be a bad PCI slot, not necessarily the card. EVGA has approved the RMA so I'll find out for certain in a couple weeks.
