Help! GTX 650 1GB Crashes

I had my GTX 650 crash, so I RMA'd it do EVGA the second one crashed, and now the third card crashed. When it crashes green lines come up on the screen then the video fails. It's obviously not the video card, so what is it.

Specs:
AMD FX 6100 Black Edition
Gigabyte MOBO (Forget Model)
600 Watt Thermaltake TR-2 PSU
8 Gigs Corsair Blue 1600 Mhz Ram
500 Gb. Hard Drive

I think it might be the monitor, which is 6 years old, or maybe that I used this hard drive in my old computer the old drivers are messing something up. It's driving me insane and I'm having trouble sleeping, please help. I have the newest Drivers for my Mobo and GPU.

I would run a memtest.

I tried the MemTest and it came out with no errors. Could it possibly be my hard drive, or my monitor. Could there being two different drivers on my hard drive mess up the video card?

You can do a clean Nvidia install if you want, or just reinstall the drivers.

So thanks for your help, I did the Driver sweeper with the program you reccomended and just got rid of all NVidia drivers. So far it hasn't crashed.
I will respond sometime soon and tell you how it goes.
