Is my GPU Failing?

Okay so I built this computer about 4 or 5 months ago and had no problems but now while I am playing games my screen will get all distorted and freeze. The GPU is brand new.
Also after this sometimes when I restart I get disk read error has occured. Any ideas?

I just bought a brand new HDD I was using my old one and just replaced it about a week ago. (I was getting the same error then) the graphics card is a saphire vapor x and it just happens randomnly on WoW and CSGO but not league of legends. How can I fix this?

Hmmmmm..... Is your rig clean of dust and not ridden with a bunch of un-civilized cables?

Its not the psu I did the test and I opened my computer and started it with the psu just plugged in and it turned on fine and fan ran... could it be my RAM? My ram is very old I reused it.

It could be that you don't have the ram to play some games. How big is your ram? And what speed?
