According to this article I found, you will not lose anything as long as you are upgrading from Windows 8. If you're upgrading from XP, Vista or 7 however, then yes, you definitely should back everything up.

Just be on the safe side and back the important things up just in case.

Here's the link to that article with more info on that: http://www.pcworld.com/article/2054670/upgrading-to-windows-8-1-your-new-os-survival-guide.html
And for info on backing-up: http://support.microsoft.com/kb/971759

This excerpt is from that first article I was talking about:

"Finally, back up your data. Microsoft provides some good advice on how to do just that, whether you’re upgrading from Windows XP, Vista, or 7.

With Windows 8, you shouldn’t have to worry about losing critical documents or files. But it’s always a good idea to store copies elsewhere, just in case. Microsoft’s own SkyDrive would do just fine."

Oh! And if you're wondering whether or not you should upgrade at all, in my opinion, I say yes! The upgrade really polishes up the Windows 8 experience and makes it the best possible version of itself.
