Is my graphics card on it's way out?

Occasionally when I wake my computer the screen stays black and I have to restart the comp. Other times the screen is "choppy" and takes a few minutes to "wake". The card is an XFX 7850

Fan is working good, cleaned all the dust out. Yesterday the screen went black while watching a video and I had to restart comp. When it powered back on the picture was choppy and took about 5 minutes to clear up
