gtx660, game crashes, black screen and reboot. need help(log attached)

After playing game like TombRaider 9 for 5 to 20 min, the moniter went black, the sound froze and then in 10 sec the system reboot. If i got lucky I could alt+tab to desktop after black screen, but most of the times it just reboot. I really appreciate it if someone can help me.

Here is sensor log by gpu-z when the system is idle and when the screen went black
(execute me for misaligned column)



Moniter: dell 2412 (1920*1200 resolution)
Graphic: gigabyte gtx 660
CPU: i5-3470
CPU cooler: cooler master hyper 212 plus
PSU: seasonic s12 ii 430w
MB: gigabyte b75m-d3h
Memory: kingston 1600 4g*2

I also did some research myself and found the following might be the culprit
1) PSU not sufficient (very likely. 660 needs 450w but i only have 430w)
2) graphic temperature too high (not very likely. 60C is not high)
3) graphic memory used up (not sure. 660 has only 1.5g effectively?)
4) driver
(i don't think cpu temperature is the reason bc it's very low when game crashes)

Could anyone help me find out which is most likely? Or is there any other reason? Thanks.

do you have the latest nvidia drivers installed? also have you patched tombraider to the latest

yes and yes
I used GeForce Experience to install the latest driver
