Faulty card or powersupply

I am running a NVidia GTX 660 TI with 1500 watt power supply. never had issues before however ever since rift merge servers I started noticing artifacts or multicolored snow in game or if I log into game screen goes white and locks pc. The card I bought in may so not sure why it would fail so quickly. it is not a problem replacing if that is the issue. Please any advice would help. below is make up of pc.
Phenom II x4 965 processor 1 MHz
memory: 8,192 (26% used)
mode: 1920x1080 with 32 bit color depth
win 7 ultimate 64 bit
On a side note I am using liquid cooling and the case is clean did that 2 weeks ago. I did notice everytime a crash happened my my graphics card fans speeds up and stays at high speed until I do a hard restart.

not sure of brand I got at fry's electronics . hard to tell though since no brand is on PSU and checked website and not one is close to one I have.

ok after thoroughly checking the sight I believe it could be a seasonic PSU 500w which I believe I had gotten a higher output I could be wrong I will see if that is issue if not then I will let everyone know.

ok so updated the power supply and not sure why I put 1500 I had a 500 or 550 but updated to a 750 yesterday after peeking the graphic cards recommended 450 watts needed. went to go into rift and still crashes. now I am getting a direct3d error only thing I can think of doing is uninstalling the driver for card and reinstalling to see if it fixes issue. * Just tried with mass effect 3 and noticed it with the game also. I am beginning to think it is either a faulty card or a bad driver.
