Screen freezes, sometimes total freeze and sometimes lag delay, might cause a reboot and i had to hard reboot to restart my PC

Hello, so i need a real help from seniors here, but my problem might be the same with other people all over the web, but this one might be a bit different.

Firstly, here is my CPU:
Windows 8 64 Bit.
Intel i5 2500k
MSI MS-7750 z68 Motherboard
NVIDIA GeForce GTX 560 Ti HAWX ( MSI )
8Gb DDR3 1333Mhz
I'm currently Using 550W Power supply

Running on Standard mode ( i didnt turn OC on my BIOS ).

At first, when i use this machine, i sometimes meet some problem that my screen flickers bad pixel with weird color laced all over the screen when im watching video, the screen freezes but i think all processes went normally, even the video itself sounded like its playing normally. Then i thought its just a normal failure so i did never check about it, sometimes this thing happened but suddenly i had to restart, but it never happens often so i dont care much about it.

around like 4 - 5 months after these ( still encountering these problems during the 4 - 5 months ), now it starts to affect my gaming, sometimes when i play game, it started to freeze the game, and my PC had to reboot by itself, and it wont start again, ( need hard reboot to be able to start it again ).

Again, i thought its just a small problem, i just let it happen and i thought it was nothing so i didn't care about it so much.

Recently, like since last month, every time i play game, its restarted every 5 mins after the gameplay ( not exactly 5 mins, but some near that. ) i checked my GPU, watched articles online, reinstalled my NVIDIA Driver, rollbacking it over and over, selected previous versions and installed it, none worked for me. i even checked my GPU fan, while gaming with max settings, it showed my GPU fan always fine, around 60 - 70, so i dont think that would be the fan problem.

So last night, i still had the same problem, and in the end i just ended up reinstall everything, i reinstalled my windows 8 and i used NVIDIA 314 driver ( old ones ) to check whether any problems would occur. Now, even i installed evrything, my windows and also my driver, my PC still reboots when i play games ( ex : DOTA2, Bioshock Infinite ) , etc.

So i tried to contact NVIDIA, but they never give me respond, tried the live chat, waited like 4 hours doing nothing, still none of their staff helped me.

So please if anyone would care enough to help me, i would request a help about my problem, whether is it my GPU , or my Power supply, or GPU Defect? or what else.. i have no idea and i really need some help!!

Checked event logs, one of the issue showed

Display driver nvlddmkm stopped responding and has successfully recovered.

please if anyone would kind enough to guide me till i find my answer, i would be really grateful. Thanks before...!!

ok, it is now 90% that the card is causing the issue. the remaining 10% is either the PSU (something went wrong with the +12V rail) or the Mobo (an issue with the PCI slot or the PCI bus in general). try installing the card on the other PCI-e3 slot (the other blue slot) and see if it freezes.

i tried connecting my monitor to motherboard, but my motherboard has no HDMI slot, so i put a D-SUB cable and connected it into my motherboard, nothing happened and i can't view anything, its all black, seems like it wont work with D-Sub, only with HDMI. i tried to select source into D-Sub from my monitor but it still nothing displayed

temps in the case is normal at the moment sir, when i use it for gaming like LOL it usually takes from 60 to 70 degrees, but still, it shuts down randomly :(

ok, turn on the PC without the monitor, an try to access any music file on your pc using your keyboard only. can you do that? i want to see whether it freezes or not. (try many time if you have to, we just need to see if the issue is with the card or elsewhere)

tried that for like 6 hours, and it doesn't freeze... i just remember every step and second needed and i did that, pc doesn't froze after i do all the step, music plays, then i start plugging onj the monitor, it plays
