New GTX 780 (screen goes black + Fans hit 100% + System reboot)

I just got a new gtx 780 and through Valley benchmark I was able to get a stable overclock of 1250mhz on the core (ran it for 2 hours with no issues).

I made a post about this earlier and I was told it was a problem with battlefield 4 and I should just wait for new drivers.

I just purchased COD ghosts and the issue is still there. I tried reducing the core clock down to 1150 and even tried under-volting the card. After playing the game (cod ghosts or BF4) for a little while my fan will hit 100% my screen will go black and my system will then either reboot or force me to manually reboot.

I even tried rolling back my Nvidia driver from 331.65 to 327.23 and the temps don't exceed 65 degrees C.

What could be causing this problem?
Please spitball some ideas my way, I cant seem to be able to solve it.

does the screen go black if you turn the fan speed up to 100 with no overclock?

Your PSU is absolutely fine:) 
Have you tried other games? Is it just COD Ghosts and BF4?
I'm thinking it might be a problem related to overclocking the GPU and maybe it's damaged but the temps are okay. What was the temperature when it was overclocked?
