PC destroying HDD's please HELP

Please HELP my pc suddenly destroyed my HDD (S.M.A.R.T. say that number of reallocated sectors starts suddenly increasing and then hdd died) I buyed new hdd and instal win 7 but after week reallocated sectors start to increasing again and HDD again died!! So i conected my 2 years old hdd which i using for backup and it also dies !! What is wrong with my PC ? I really dont know what to do and I need to use my PC. THANKS FOR HELP!

And my HDD's are Seagate LP ST31500541AS 1,5 TB
Seagate Baracuda ST3500418AS 500 GB
And my old HDD Seagate Momentus ST9160821AS 160GB

CPU: Intel e5700 3 GHz
GPU: Asus radeon hd 5770
RAM: 3 Gb

And the PSU is about 4 years old and heavy used so i think it can be bad.

One more question is: How can I be sure that PSU is the problem because if i buy new and my problem doesn't be solved what i can do ? What else can cause this HDD dying ? I would rather buy new PC but no moneys left for it.

16v? holy crap. unplug the power from that pc and never use it again until you replaced the power supply. Nothing on a motherboard is suppose to use more than 12v so anything above that will start to cause problems eventually. 16v is way above
