Assistance with choosing a new graphics card

Hello,
i want to upgrade my graphic card because i noticed that my current one can't handle new games (meanwhile Assassins Creed 4 and Call of duty Ghosts).
my graphics card is nvidia geforce gts 250, and i want to find a new graphics card within the price range of 250-400$, the new card needs to handle the newst games on high quality and will be able to run games that will come out in couple of years from now.
here is s my pc:
CPU:
AMD Phenom(tm) II X4

Motherboard:
Gigabyte GA-MA770-UD3 v2.0 (2 PCI, 4 PCI-E x1, 1 PCI-E x16, 4 DDR2 DIMM, Audio, Gigabit LAN, IEEE-1394

Graphic card:
NVIDIA GeForce GTS 250

Computer:
ACPI x64 based PC

Power supply:
zalman power supply:zm500gt

my knowledge on the subject is very shallow so i rather that you direct me to a graphic card that will solve my problem

ill add that to the list so tommorow i can check how much it cost in my local store.
if i understand your question correctly my power supply is:
zalman power supply:zm500gt
but ill probably upgrade that too for a 750W(not zalman probably) one, like the guy in my local store recommended.
so what your saying is to go for Radeon R9 instead of nvidia cards?

i bought a less powerful model than you recommended, for a cheaper price, but it is still a workhorse.
here is the graphic card i bought:
MSI Radeon R9 280X GAMING 3GB-5.
i didn't get to try it yet but i'm pretty sure its the right choice.
i wanted to say thank you very much once more for all of your help.much appreciated.
