HD 4870 problem

Hello,
I have intel e6420, mobo GA-EG41MF-US2H (rev. 1.0), psu 500w
8 days ago i got hd 4870 toxic vapor x (512mb), and it worked perfectly.
But today after like one hour of using it started to make this

does anybody know what that means?
the same problem appeared in desktop after booting windows. i uninstalled the drivers and wanted to reinstall them but after the uninstall and after the booting which looks like on the photo the monitor goes black(into power saving mode)...
Please help.

DVI, if i take the card away from pcie and connect the vga cable from onboard to monitor, it works without problems.(im using it that way now)
here is how the desktop looks like with the monitor connected to the DVi port on hd 4870

Take the side panel off and see if the fan is turning, it does not take long for a card to overheat if the fan isn't working and this may be the cause of the artifacts.
If you see those chequerboard patterns during POST or the POST splash screen, the card is faulty.

fan is working, and it automatically spins faster or slower, depending on the thing im actually doing, so this seems fine to me.
Tried different booting, tried driver sweeper like 5 times, reinstalled drivers like 7 times.
finally it is working now without the patterns, but i CANT OPEN CCC, it seems to me like the card is all right but the drivers make those fails...
does anybody know the solution with the non-responsing CCC? (double clicking it does nothing, right click desktop and select CCC nothing, run mom.exe nothing.
i have win xp 32bit

ok so in indle(desktop, internet browsing etc) the card is ok, no patterns or anything, 42-45 °C at 1700RPM, i pulled it to 250RPM to heat it a little more to see if that causes it, at 51°C nothing, so i pulled it back to 1700 RPM
next thing, i started a game (WOT) after few minutes there is slightly visible the pattern, (48-51°C, 3000RPM) after few more minutes the pattern is more and more visible, after switching to desktop(alt +tab) the pattern goes off in like 2 mins (temp 42-45°C again 1700rpm)
this is really weird

it does also in other games
could it be psu? like its draining too much power? but it was 7 days okey, even with 8-9 hours straight playing....
 
Check the CPU temperature as well but it I'm certain the card is on its way out, time for a warranty/Ebay return.
How did you change the fan speed?
If you're using something like Afterburner you can try underclocking the card. If that helps, or eliminates the problem it's !00% the card and time to return it.

today update
i ran VCORE memory test, 0 errors. I unplugged 6pins, and made me sure they are connected correctly, made me sure the card is correctly seated in pcie, so then i underclocked the GPU from 780MHZ to 770MHZ. Then started game, and the patterns were gone.
i really dont know what is happening. However it works and i think the 10mhz wont be noticable.
Hope it wont start make some crazy sh*t again.
