well i have a top mounted PSU which sucks air in from below and flushes it throught the back.

im currently using a stock cpu but going to upgrade it to evo 212.

and i have a rear fan just next to the cpu.

my gpu is 7870 with fans facing down. it is idling at 36-38C. (problem?)

how is the airflow pattern here? good? bad?
i do not have any front intake fans as my case does not support it.
do i need an intake fan?

i do have an extra fan which i can install on the side panel of the case (the side which is take off in the pic)
should i put that extra fan on the botton of the side panel or in the middle. And should i make it intake or exhaust?

typically, intake fans are positioned at the front, side and/or bottom. Exhaust is usually at the rear and/or top. Since you have no front intake available, I would add the fan to the side panel and have it as an intake (blowing outside air into the case). As far as location (bottom/middle) it may depend on if either location would cause interference with existing or future components.
