is my graphics card broken?

im having some difficulties with my pc and dont know what to make of it!
It seems to crash for any game i play (problem has been getting progressively worst). Every time i try to play any game (Battlefield 3, 4, State of Decay, Batman).
What happens is that my pc screen goes black, gpu fan goes into overdrive, can still hear sound for a few seconds afterwards, thereafter goes into a somewhat high pitched sound and i have to reboot by switching off at the power switch.

When i first encountered this about a month ago i could maybe get 1/2 hour of playing in, now i can be lucky if the game even starts!

System Spec:
Motherboard: Gigabyte H55M-UD2H
GPU: Gigabyte Radeon HD 5850
Ram: 2x Corsair 2048mb ddr-1333 sdram
Sound Card: Creative SB X-Fi (installed this old creative card after i discovered on internet how many others having same problem like me was related to Realtek onboard)
Win 7 64bit

i have thus far updated:
1) gpu drivers - Catalystâ„¢ 13.9 for Win7 (64bit),
2)mobo bios - F11

Points worth mentioning:
-i have run memtest for 6hrs, no errors (should i have run it longer?)

-i initially thought it was my ram since i alternated taking out each of my 2 sticks and for some reason while the 1 stick was removed my pc seemed to run fine (could play games for about 4hrs straight) for about 2 days then slowly the symptoms reappeared!

-one other point worth mentioning about my ram is that i have a Massive CPU Fan contraption (Coolermaster V6 ?). And the pipes extent so far outwards that it touches my 1st ram stick!!

-uhm?,...ran Furmark, sometimes it fails sometimes it passes but must admit i dont know what im doing in this app and more so what it actually means when you fail or pass a test!
-wont be very surprised if it is my psu, since its an old psu cos my gigabyte shorted out when i powered on pc and died about 2 yrs ago. dont even know what spec my current psu is
-no overclocking whatsoever! Never have and never will

thats about all i can think of now.

Please advise as im lost!

Sounds like the gpu it's over heating. What temps ate you getting when gaming?

As shadow32 states your graphics card appears to be overheating. It may be clogged with dust, try to blowout the dust with compressed air or a product like Endust for electronics.
