Freezes on a load, occasionally

I built this computer about a year and a half ago and I've had this issue for the past year or so. But I'm finally fed up with it. When the graphics card is under a load (playing videos/gaming) the computer will screen freeze anywhere from 10 seconds to 2 minutes.
Computer specs are:
i5-2500k cpu
16gb Gskill DDR3-1600 ram (4x4gb)
Gigabyte Z68XP-UD3 MB
Windows home 64bit
Nvidia GeForce GTX570
120gb SSD
500GB HD

Dell XPS630 case with a 760W Corsair PSU

I've tried another GPU and I had the same issue, I tried reinstalling windows and still have the same issue as well, so I think I've ruled out a software issue ( I think )

Lastly, this is the last warning I received on event viewer
Display driver nvlddmkm stopped responding and has successfully recovered

Temps are in the 45* range.

Please help...

it could be you're overworking your CPU. Other than that, it could be that both GPUs were too weak to run the game/games you have been playing.

I appreciate the reply, but i cant see how i would be overworking the cpu or gpu. I havent even tried games in a while, it is currently freezing on youtube videos if im doing anything else while the video is playing.

 
could be power supply problem. the freeze only happen when your pc fully loaded like playing games. i also got freezing in the past due to PSU cannot provide enough power to the pc. in my case i was using GTS250 with some generic PSU. the freezing only happen when i run crysis. for less demanding games it is fine. replace my PSU with more quality unit solve the problem entirely. i bet the PSU that included in your PC is a good one but yours could be faulty.
 
Yes, they provide some cheap low quality PSU's with cases. Next time when you buy a case, don't go for bundled PSU.
Can you provide the details on the PSU you got with the case? What brand? Amps on the different rails? Model?
Otherwise, get a picture with a camera and post it here.

Okay, so I can rule out the PSU. I replaced my old PSU with a Corsair ATX760i and it still freezes so now I'm stumped. Not sure what the issue is.

Tried another video card, still froze
Reloaded windows, still froze
Replaced PSU, still froze.

The GPU drivers are up to date, I could try a different pci-e slot to see if it makes a difference.

The bios version date I have is 10/11/2011, not sure if updating will fix my problem, but it seems like some steps are involved.
