I'm going to get a new graphical card that uses 20 watts more than my old card.
Am I supposed to get a new power supply now?

This is a medion PC and there is no way to get specs on certain parts so I don't know how many watts my current power supply outputs. However I'm wondering if I should get a new PSU for 20 watts difference. It's said that the Club3D Radeon HD 7790 '13Series requires 450 Watts to function though. Does that mean 450 Watt has to be free at all times?

You can look at your power supply and it will tell you the specs on it... there pretty much has to be a sticker on it in order to be compliant with most laws.

Thanks. It's 400 Watt. So 50 short. If I get a 500 watt PSU would that be ok?

yes but I would get a pretty good one, if you're in the US.. usually I'd recommend something cheaper, but its on sale as of 18 mins ago so...

Possibly the best PS that exists at that range of watts, and its discounted severely today.

I don't live in the US. Is there any difference between ATX and SFX?

SFX means small formfactor... meaning unless you have a specific case the PSU needs to be ATX (nearly all cases are)
