Windows / CCC Not Allowing 3 Screens

I recently bought a MiniDP > HDMI adapter to connect my PC to my TV, which would make this my 3rd display connected.

I connected the cable, and tried to extend the display through "screen resolution" on the desktop. When I clicked "apply" it gave me an error: "Unable to save display settings". Given the error, I decided to reinstall drivers. I got the latest from the AMD website after completely uninstalling what I had, and it still didn't work.

I went to CCC > Desktop management > Creating and Arranging Desktops and attempted to extend the display from there, to no avail. I needed to disable one of my current monitors to enable it.

I have:
ASUS PB278 2560 x 1440 60Hz <HDMI>
DELL E196FP 1280 x 1024 @ 60Hz <DVI>
And I am trying to connect a Panasonic Viera <MiniDP to HDMI>

I am running these from a AMD 7970Ghz.

I've swapped the cables around etc. with no success, and am out of ideas. I'm hoping somebody here can help.

I will post pictures when I get access to a camera, which will be soon.

Is the adapter active or passive? In order to run three screens you need a DisplayPort cable or a active DisplayPort Adapter even though you are using a adapter if it is not active it will not work for Eyefinity.

Well there's my problem. I have a passive adapter.

I can use any 2 of the 3 screens at a time, so I am assuming that there is some kind of lock on using 3 screens with an adapter? If so, is there any way to remove that lock?
