Alright, here is my setup:
-AMD FX 8350 Stock
-Saphire 7850 1GB OC Edition
-ASUS M5A97 Mobo
-8GB RAM
-XFX 550 PSU
-CPU FAN is the Hyper Evo 212 (i don't remember) - If I remember correctly it did not fit 100% so its kinda jerry rigged on.
*Note* I also have the side of my computer open and put a desk fan on it so that it gets more air flow

The problem:
I will be doing something at not even Maxish load, for instance just now I loaded CS:GO and within 10 Minutes I crashed, I crash during Starcraft 2, I crash in League of Legends, I crash in Minecraft, I crash in everything... It really sucks to be honest.
Their is no crash screen. it just turns off, black, not even a shutdown screen, nothing, just off.

The Idea:
This is where I need help, I am not sure if the CPU is getting too hot,
or if the Graphics card is getting too hot.
I also thought it might be my PSU being pushed to Voltage,
Whatever it is I am sure more air flow wont hurt, lol.

I wasn't sure where to place this so its in GPU !
If you can help, thank you very much!

check the cpu temps with core temp and or hw monitor - this cehcks also gpu temps... i suspect the cpu overheats considering you had issues installing the cooler.

^^ Agree with the above. If you were not able to secure the Hyper EVO 212 properly, this could be causing your CPU to overheat and shut down to prevent damage.

Get that fan/heatsink on properly, with thermal paste applied correctly (many guides here on Tom's, recently updated).
